Notes For Patients

The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards, applicable laws and state and federal record retention requirements.The retention period for your records will be the greater of seven years after the last date of service for a patient, or until the patient turns 18 years of age. A patient's records will be destroyed after the applicable retention period.
